Implant and Surgical Referrals
Implant and oral surgery referrals, planned with in-house CBCT.
We accept referrals for dental implants and oral surgery from dentists across Surrey. Every implant case is planned using our in-house CBCT. Where you’d like to restore the implant yourself, tell us on the form and we’ll plan around that.
Cases we accept
- Single-tooth implants
- All-on-4 / All-on-6 full-arch treatment
- Implant-secured dentures
- Bone grafting
- Extractions, including surgical extractions
- Second opinions
What to include with your referral
- Your GDC number, practice address and contact details
- Your patient’s contact details — we contact them directly to arrange an appointment
- Relevant medical history, including medications and allergies
- The reason for referral and the teeth involved
- Any treatment you will carry out yourself, such as restoring an implant
- Any radiographs or reports (JPG, PNG, PDF, DCM or ZIP, up to 20 MB)
